The feats happened.

Deal with it.
 
If it happens once, it's an outlier.

If it happens twice, it's debatable.

If it happens more than that, it's normal for the character (if potentially their upper limit) and should be treated as such.

You are wrong, if there's more feats of a character going less than mach than LS, then they are only less than mach goddamnit!

I mean, if something happens only once and nothing strongly contradicts it, its not an outlier

Likewise if something happens twice and nothing strongly contradicts it, its not debatable.

Like if someone perform a somersault only once and never do it again, its not an outlier to say s/he can somersault, unless s/he specifically mention s/he can't really do that and just got lucky that 1 time or any other thing that oppose the feat.

He tightly clenched his right fist and used his entire body weight to thrust it forward. No one would have been able to keep up with its overwhelming speed. Their bodies would have been smashed to pieces before they could even hope to follow the tip of the lance with their eyes. But the experience he had built up accurately guided his movements. He had achieved a surefire attack that would work on Magic God Othinus and no one else. Kamijou Touma’s right fist was sucked in toward the one point that no one else would have been able to reach. His fist reached the sharp tip of Gungnir. In that instant, all sound vanished from the world. The kind world vanished and everything was dyed in black.

Two more examples of Touma reacting to shit that's magnitudes faster than light itself, since Flaming;Sword is both compared to Gungnir in speed/power (at the least), and is released from infinite acceleration after descending down the Sephiroth.

There's a third one I could post of him reacting to Flaming;Sword in the previous instance that's mentioned in the quote, but I'm lazy and you all get the point by now.

His strongest punch, that went back in time and wrecked the weakest Garou but still left him more than alive barely kicked up gravel

So that one’s more fun to play with

Flipping Io’s Surface, the Jupiter Sneeze, whatever the fuck happened with the punch^2 are the only notable feats from that fight. The rest are trash little better in terms of CoE than the one you posted or I mentioned. Weak ass nuke fist in the vacuum of space not withstanding either. No atmosphere, no shock wave, energy is fully dependent on heat and brightness to be measured.

Space is empty and vast, no shit

Problem is its empty, vast, and every part of it is in constant motion of some such

In a static system, it'd be pretty simple to reach your target without automatic assistance across those distances as you're not confounded by moment to moment alterations of position across space and time for literally everything that exists.

Our planet rotates around its axis, which revolves around the Sun, the entire star system revolves itself around the galactic core while every bit of matter that exists appears to be screaming away from each other. Even within our local Earth to Moon system, we need to account for the confounds of our planet's rotation and the Moon's orbit to not miss that 0.5 degree dot in the sky. We're already matching the relative velocity, try reaching your target from outside that system, matching that relative velocity, and not over shooting your target without sufficient mental processing speed to accomplish the task. Accelerating and decelerating aren't going to cut it. Sight ain't helping you, light is the speed limit there. Sensory bullshit is dependent on your ability to process the information, tied to your thought speed. Have fun playing the worst and most empty game of "find the needle in the haystack" if you can't some what measure up to how fast your scream across the uncaring void.

Same issue with teleportation as a power being overly simplified. Or how time stop as portrayed needs to always be universal as local would have that chunk stuck out in the vacuum of space after a moment in time of further movement.

Maki can move in mid air and can slice through or tank basically anything Toph throws at her.

Pretty damn sure Toph can murk the fuck out of Maki considering we explicitly see Toph be able to bend huge goddamned Earth Statues in Book 2 when she, Aang and the rest invaded the Earth Kingdom's Palace? And those was thrown at her in flight, ain't shit Maki is doing against that.
