That's literally how it works.
Where or how Cu thrusts the Spear ceases to be a formality the second he says Gae Bolg and wants to target the person. Aim at the ground, behind him or even at himself, the Spear will pierce the opponents' chest because "The Effect has already happened".
Even if you go back in time and kill Cu or kill his Master, the Effect will remain in place and you will die right then and there when it goes right back to that moment.

Cu can still enhance his Runes to get past God Hand atleast once in canon and in the UBW Anime, Cu was able to use the Thrust of Gae Bolg despite not being in range of Saber like he was in the Manga.
It's basically the closest thing to an Authority of Cu so it's a bit more complicated than that.
